Watch this video to learn how this moment is yours As part of the palliative care CNS team, you will be working collaboratively with other palliative oncology hub members and other health professionals to provide coordinated, tailored care to patients, family, whanau and caregivers. This hub is a nurturing and supportive team environment. You will also provide specialized education and mentoring of other health service professionals.
You will be involved with initiating and or participating in evidence-based service improvements, quality enhancement, policy development and building of staff capability. Aspects of this role include working closely with patients and their whnau across the health sector in a connected and inter-professional manner. You will work with palliative care patients and their family to optimize their journey, working closely with primary care providers, hospice, aged residential care, and secondary care providers.
Key Requirements Include: Nursing Council of NZ registration, and current APC A relevant postgraduate qualification (NZQA Level 8) - or be willing to work towards this qualification Significant relevant clinical experience Be able to work collaboratively with health professionals and as an autonomous practitioner. Excellent clinical assessment skills Current full New Zealand Divers License Why choose South Canterbury DHB?
