Right now, we're looking for an Occupational Therapist with community experience to join the Primary Health Partnership, Rapid Response team.
This service provides a multidisciplinary approach to patient care and is focused on, hospital avoidance, early supportive discharge (from hospital) and non-acute rehabilitation in a homecare setting The Occupational Therapist(s) in this team work collaboratively with the Emergency Department, General Practice, Residential Aged Care and are part of a team that includes, Registered Nurses, Physiotherapists and the Health Care Assistants work under the delegation of the Healthcare professionals. This role will suit someone who is able to work with a wide group of healthcare professionals, have the ability to practice independently and use prior expertise to ensure patient and client centered care enabling them to maximize independence. To be eligible for this role, New Zealand Registration and a current Annual Practising Certificate is required along with a current full driver license.
Other requirements for success in this role includes: Previous community experience and/or a passion for providing care that supports people in their own homes Ability to prioritise workload, supervise and delegate to care staff Delivery of a safe and effective patient focused service Have the ability to work with patients and their whanau and family Have a personal commitment to professional nursing development and continuous quality improvement.
