With a large hospital campus in Palmerston North, it also provides key health care and support services to neighbouring communities. The Opportunity We have an exciting opportunity for a Registered Health Professional to help us develop new ways of working to assist people requiring a support needs assessment and to help us transform the way we engage with our community partners.
In this pivotal role it is imperative you are recovery focused and can recommend solutions for people to achieve maximum independence and participate in society wherever they may live. Te Uru Rauhi - Mental Health & Addiction Services provide a lifespan approach across our community districts of Manawatu, Horowhenua and Tararua in partnership with Kaupapa Mori Oranga Hinengaro services and the acute inpatient facility. Using the interRAI clinical assessment tools (willing to train if required) you will be working in partnership with people using services to determine the current abilities, resources, goals and needs of a person in relation to supported accommodation and packages of care.
The support needs assessor will work primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged. To be successful you will have the following competencies: Knowledge around Mental Health Addiction Services internally & externally Assessment skills to identify the needs of people using services and provide an accurate summary of the persons situation. knowledge of available community supports and services.
An ability to work within a changing health environment A mana enhancing and restorative approach within your practice Knowledge and working relationships with iwi/Mori providers in the district Knowledge of community resources and networks An ability to adapt and flex to trial new ways of working A strong desire to help clients and whnau achieve their goals Effective written, oral and interpersonal skills with an ability to communicate with people from a wide variety of backgrounds and cultures Strong organisational and time management skills Skills to use a laptop and learn new assessment software A full, current, driver's licence Great communicator and equity at the forefront of decision making.
