He krero m te tranga | The Role Community Support Worker's with Access and Choice aim to connect directly with whaiora who are referred via their GP practice. This involves working alongside whaiora at their GP practice to develop and complete an identified goal for up to three months to improve their health and wellbeing. This role is walking alongside whanau to give them the tools, confidence, knowledge, and guidance of their community.

For example, navigating social services, developing personal plans, gaining employment, budgeting, and housing. You will also be an integral part of the general practice team and working in collaboration with other community partners, as well as building new relationships through networking. This is a permanent part-time position, working 32 hours per week, Monday - Friday.

You will be working across the Hutt Valley and Wellington area. Duties of the role could include, not limited to: The role involves working alongside whaiora at their GP practice to develop and complete an identified goal for up to three months to improve their health and wellbeing. You will be an integral part of the general practice team, working directly with individuals and their families to help with goal setting, and then connecting them with community services and support agencies.

This role involves walking alongside whanau to give them the tools, confidence, knowledge, and guidance of their community. While you will work to support patients of all ages, you'll predominately support people from the ages of 18 and above. Ensure key documentation and handovers are accurately recorded.

Attend and contribute to team meetings, bringing ideas and updating others on their activities. He krero mu | About You We are looking for someone who is positive, flexible, vibrant, confident, and compassionate to work in our service. You'll use your non-judgemental attitude and belief to motivate and encourage whnau realise their potential.

You'll have the ability to easily build professional relationships, while maintaining strong professional boundaries To be successful in this role, you will need: A New Zealand Certificate in Health and Wellbeing - L4 or higher level of mental health support work (or equivalent). Experience working with a recovery-focused approach to ensure the delivery of quality service to our whnau. To be able to demonstrate experience supporting whnau with challenging behaviour and provide a calm and patient approach to behavioural challenges you may face.
