Te whiwhinga mahi: The opportunity We have an exciting opportunity for a Clinical Pharmacist - Prescribing (0.8 FTE) based in Taup. This role will work within our Extended Care team and is your opportunity to utilise your pharmacotherapy skills and work in a supportive and collaborative environment, working to eliminate inequities and achieving best possible health-related outcomes.
You will assist patients and their health care providers in optimising medication use, reducing/preventing medication-related problems, and improving health outcomes by providing medication therapy management and other pharmaceutical care services. You will also promote health and wellness education, and disease prevention services. This role in the flexible and future-focused extended care team works alongside iwi, general practice, community health providers and Te Whatu Ora teams to provide quality evidenced based clinical care.
He krero mu: About you Ideally you will: Hold a current annual practising certificate (APC). Have a minimum of 2 years experience working in a clinical environment, and 5 years post-graduate experience. Hold a postgraduate diploma in pharmacist prescribing (preferred).
