6 FTE 24 hours a week role is part time, with a level of flexibility in Kapiti. As a member of our Kapiti Pathways' Access and Choice team, you'll be under the guidance of the Team Coach. Collaborating with a dedicated group of Health Coaches/Support workers, your role will involve working within the community.
Utilizing your expertise as a skilled practitioner, you'll assist individuals in setting goals and implementing action plans. You must hold a current full drivers license. Hold a current first aid certificate or be willing to obtain one. Hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.
