Registered Health Professional Position Description Lifewise is an innovative organisation working together with people who are often marginalised to create better services and solutions to enable their full participation and ability to thrive . Lifewise Mental Health & Addictions is a 14-bed supported accommodation residential service that has been based in Rotorua for over 30 years formally known as Bainbridge. The service is an embedded supported accommodation provider that provides 24/7 care to people experiencing distress and addiction issues.
The therapeutic environment ensures safety for all people in care with a diverse workforce that is guided by sound clinical leadership. The heart of the practice is people and Lifewise has a proven track record of Peer and Lived experience at all levels of leadership to ensure our work is authentic. Working under the principals of Te Ara Tauwhirotanga (the pathway to kindness, Lakes DHB) each of the three concepts (Tangata, Taiao and Tikanga) must be represented in the Lifewise provision of care.
The team has been innovating with this model and deliver via daily onsite activity programmes. Co-ordinate and conduct intake/admission process for each service upon acceptance of referrals. Develop wellness safety assessment plans for residents. Conduct resident reviews at timely intervals.
Support or lead clinical handovers, to ensure staff are fully informed of resident needs. Attend discharge planning meetings and multi-disciplinary meetings when required. Conduct presentations on behalf of Lifewise Rotorua to external stakeholders when required.
Lead monthly Quality meetings, investigate and report on Quality matters monthly. Manage medication management process, including updating relevant policies, conducting medication audits and completing medication competencies for self and other staff. Conduct Internal Audits as per the audit schedule.
Complete monthly reports of activities Co-ordinate meetings that involve the clinical care and support for clients Understand the position of Mori as the tangata whenua.
