A Community Facilitator works alongside our disabled partners to enable good lives, drive normal life outcomes and to encourage our partners to achieve their dreams We are seeking a compassionate and flexible Permanent Reliever to provide cover in various residential homes.

You will need to be able to adapt quickly to different care environments, working in various homes for varying durations based on ConneXus rostering requirements. This role offers guaranteed minimum hours with set days but requires flexibility to work varying start and finish times. The role may include but is not limited to: Supporting our disabled partners to develop new hobbies, skills, and interests.

Helping with household tasks, such as gardening, cleaning, and meal preparation Providing personal cares (toileting and showering) Encouraging our disabled partners to actively engage within our communities. Administering medication and providing support to and from appointments Maintain accurate records of the care provided, including daily notes and any incident reports. Being encouraging and supportive of life skills, such as budgeting and pursuing employment opportunities.
